APPENDIX B West Jersey Manufacturing Company Docket No. 99900816/87-01 NOTICE OF NONCONFORMANCE .

Based on the results of an NRC inspection conducted on January 12-14, 1987 it appears that certain of your activities were not conducted in accordance with l NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these reauirements are as follows:

1. Section 1.1 of No. 1.0 " Introduction" of the Nuclear Quality Assurance Program Manual (NQAPM), Revision a states, in part, "The intent of this manual is to... meet the standards of ASME Section III, NCA-3800."

Subsection NCA 3866.3 of Section III of the ASME Code states, in part,

" Documents for procurement of material and subcontracted services shall include requirements...to assure their compliance with this Section."

Section 4.1.2 of No. 4.0 " Procurement Document Control" of the NOAPM states, in part, "The purchase order shall require a vendor...to certify on the CMTR that the material was furnished, processed...to a Quality System Program... meeting the requirements of ASME Section III NCA-3800...."

Contrary to the above, a Quality Systen Program was not invoked on P0s 5366, 4911, 5322, 4900, and 5359 to Ramball Test Laboratory for analytical testing of nuclear material; and 4968 to Pan Am Forge & Tube for nuclear forgings. (87-01-02)

2. Section 4.2.1 of No. 4 " Procurement Document Control" of the NOAPM states, in part, " Purchase orders will be placed only with those companies shown on the approved list...."

Section 7.3 of No. 7 " Vendor Approval" of the NOAPM states, in part,

" Audits of Paterial Vendors shall be performed at least each 12 months...."

Contrary to the above, P0s were placed with Donovan Heat Treating in December 1986 (5628) and November 1985(5326) and Dallas Forge (DF) in December 1983(4353), but neither company was on the Approved Vendor List (AVL) when the order was placed; and documented evidence was missing to verify that DF had been audited. (87-01-03) ,

3. Subsection NCA-3867.4 of Section III of the ASME Code states, in part, "The Certified Material Test Report shall include the actual results of all required chemical analyses, tests...."

Section 4.4.5.1 of No. 4 " Procurement Document Control" of the NQAPM states, in part, "The West Jersey CMTR shall contain the actual results of all required tests, chemical analyses and examinations."

Section 17.6 of No.17.0 " Quality Assurance Records" of the NQAPM states, in part, "...the following information shall be included...on the material test report...: heat treatment data...."

Contrary to the above, it was noted on nine nuclear orders that data on WJM and DF and Pan Am Forge & Tube CMTRs differed for: (87-01-04)
