e APPENDIX B Limitorque Corporation Docket No. 99900100/86-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on September 22-26, 1986, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting auality shall be prescribed by documented instructions, procedures or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings. Instructions, procedures or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

1. Subsection A.1 of Section I of the Quality Assurance Manual (QAM) dated July 11, 1986, states, in part, "It is the intent of this manual...to meet the applicable provisions of... ANSI N45.2 as they pertain to Limitorque's products."

Section 5 of ANSI N45.2-1977 states, in part, ". . . procurement documents shall require contractors [ vendors] to provide a Quality Assurance Program consistent with the pertinent requirements of this standard."

Contrary to the above, it was noted that Limitorque failed to impose the recuirements of a QA Program (ref. P0 No. 3M4515 dated January 28,1985) upon H. K. Porter for nine Class 1E DC mctors which were used on safety-related operators sent to Portland General Electric Company for use at the Trojan facility. (86-01-03)

2. Section 6 of ANSI N45.2-1977 states, in part, " Activities affecting quality shall be prescribed by documented instructions, procedures...of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures...."

Contrary to the above, written instructions / procedures did not exist for current practices followed by the (a) Parts Department to ensure correctness and completeness of spare part orders, and (b) Parts Department, Application Engineering, and Quality Control to initiate, review, and issue Certificates of Compliance (CofC) for spare part orders. 3. Section 2 of ANSI N45.2-1977 states, in part, "The program shall provide for the accomplishment of activities affecting quality under suitably controlled conditions. Controlled conditions include... assurance that prerequisites for the given activity have been satisfied."

Contrary to the above, Limitorque did not process Arkansas Power and Light (AP&L) Company P0 No. 51965 for terminal strips under controlled conditions to assure that prereouisites affecting auality of the order were identified and accomplished. It was noted that Limitorque incorrectly advised AP&L personnel that Buchanan 724 terminal strips were a correct replacement for qualified Buchanan 524 terminal strips, and Limitorque issued a CofC that certified that Buchanan 724 terminal strips were environmentally qualified in accordance with tests documented in Limitorque Report B0119, Revision 1 dated November 10, 1982. As a result, AP&L was supplied with Buchanan 724 terminal strips that were, in fact, not qualified. (86-01-05) l l
