NOTICE OF NONCONFORMANCE Based on the results of the NRC inspection conducted on May 30 to June 4,1986, 4 it appears that GGFF activities were not conducted in accordance with NRC re-quirements.
l Criterion II of Appendix B to 10 CFR Part 50 states, in part, "...shall esta-blish...a quality assurance program which complies with the requirements of l
this appendix. This program shall be documented by written... procedures, or instructions...."
Criterion V of Appendix B to 10 CFR Part 50 states, in part, " Activities af-fecting quality shall be prescribed by documented instructions, procedures...."
Subsection NCA-3851 of Section III of the ASME Code states, in part, "The Material Manufacturer...shall establish a Quality System Program for the control of the quality during manufacture...."
Subsection NCA-3862 of Section III of the ASME Code states, in part, "The Program shall be documented...and shall be implemented by procedures...."
Contrary to the above, GGFF manufactured and certified ASME Section III, Class i 1 flanges without having a documented quality assurance program. (86-01-03) i 4
