s NOTICE OF NONCONFORMANCE i

Based on the results of the NPC inspection conducted on April 22-25,1986, i t appears that certain of your activities were not conducted in accordance with

NRC requirements. s

! A. Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affec-l' ting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be ac-complished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantita-

' tive or qualitative acceptance criteria for determining that important i activities have been satisfactorily accomplished." ,

Nonconformances with these requirements are as follows: -

4 i 1. Section 4.0 of Laboratory Procedure 012-R1 states, in part, "The j temperature range for operation and calibration of the spectrometer is 38*C i 2*C."

Contrary to the above, the temperature of the spectrometer unit when observed on April 24, 1986 was 22.3 C. The unit was, in all other i i, respects, ready for operation. (86-01-02)

2. Route sheet (resume) NPG-0R-215271-NUC states that globe valves to f be supplied to Farley Nuclear Plant will be tested using Henry Vogt procedure NPG-NDT-016-1.

Contrary to the above, the NDE inspection test reports for these

• valves show test procedure NPG-NDT-016-2 was performed. No evidence was presented to show that a resume change sheet was prepared to document the reason or acceptabili.ty of the procedure substituted.

Contrary to the above, quality program audits have not been performed to date for vendors supplying calibration services to the Henry Vogt Machine Company. In addition to not being audited, one of the vendors who has supplied calibration services was not found on the approved vendors list.
