APPENDIX A Georgia Institute of Technology 99900903/85-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on December 20, 1985, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Nonconformances with these requirements were as follows:

A.

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactnrily accomplished."

Contrary to the above, activities affecting quality, to wit:

determination of test sample irradiation dose rate, exposure time and total dose, were conducted without documented instructions or procedures.

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Paragraph IV of the Georgia Institute of Technology Quality Assurance Marual for Hot Cell Operations states, in part:

" Documentation will be acquired and maintained to show NBS traceability of the calibration for equipment used to measure radiation dose...."

Contrary to the above, there was no documentation relating to the calibration of the clock normally used to measure sample radiation exposure time and thus, with dose rate, to measure radiation dose.
