APPENDIX A Exxon Nuclear Company Docket No. 99900081/86-01 NOTICE OF NONCONFORMANCE During an inspection conducted October 27-30, 1986, fabrication and testing of fuel assemblies, computer code design activities and the implementation of the Quality Assurance (CA) Program at the Exxon Nuclear Fuels Department facility in Richland, Washington were reviewed. The QA program requirements are discus-sed in the Exxon Nuclear Company, Incorporated Quality Assurance Program Topical Report for Nuclear Fuel Design and Fabrication XN-NF-1A, Revision 7, dated January 1985. This program includes computer code design activities and activities associated with the fabrication of fuel assembly components manu-factured at the Richland facility. Section 3.0 of XN-NF-1A, Revision 7, requires, in part, that QA for design include independent verification of calculations in accordance with Design QA Procedures. Section 9.0 of XN-NF-1A, Revision 7, requires, in part, that radiography and ultrasonic testing be subject to special process cualification which requires the procedures to be reviewed and approved. As discussed below one example was identified in which a procedure was not adequately implemented and two examples were identified in which unapproved procedures were used.

Based on the results of this inspection it appears that certain activities at the Richland facility were not conducted in accordance with commitments made to the NRC. These items are set forth below and have been identified and classified in accordance with the NRC Enforcement Policy,10 CFR Part 2, Appendix C.

1. Section 1.2.12 of Quality Assurance Procedure XN-NF P00,002 defines code verification as that process by which one ascertains the equiva-lence of the theoretical and/or a priori basis of a computer program with the actual code written.

Contrary to the above, the verification calculations performed to support the addition of the Moody critical flow model in the RELAP5/M002 computer code (version U0CT85) did not adequately test the implementation of the new modelling with the theoretical basis.

2. Section 9 of XN-NF-1A, Revision 7, requires, in part, that radiographic tests applicable to fuels manufacturing be subject to special process qualification which requires the procedures to be reviewed and approved.

Contrary to the above, the operator of the x-ray machine used an unapproved procedure to perform fluoroscopy on fuel rods in the manual mode. (86-01-02) .

3. Section 9 of XN-NF-1A, Revision 7, requires, in part that ultrasonic tests (UT) applicable to fuels manufacturing be subject to special process qualification which requires the procedures to be reviewed and approved.

Contrary to the above, the operator used an unapproved coordinates chart to read the values on a current UT trace being used for the zero settings on a Gould ES 1000 recorder. (86-01-03) l
