i. This refers to a meeting held with Messrs. E. E. Van Brunt, Jr., J. G. Haynes,

'and other members of your staff at the NRC Region V offices in Walnut Creek,

California,'on December 17, 1986. The subjects discussed are susuarized in

[ Meeting Report Nos. 50-528/86-38, 50-529/86-38, and 50-530/86-32, enclosed

j herewith for. your. infonnation.

The bulk of the discussion revolved around the response by APS people to

.

operating events.. It was emphasized that operating personnel are still going

through a period of attitude formation. Attitudes formed now will endure for

i' years and be hard to change later. It is very important that a self-critical i

diagnostic approach be taken to all operating events. Several examples were

! discussed where operating perscnnel could have been more cautious and

i thorough on determining root ~cause. Operation should not proceed until

j causes are thoroughly understood,

! Finally, several events discussed during this meeting indicate that some of  !

! the APS staff does not fully appreciate the caution required to work on

i radiologically controlled systems.
