Due to an administrative error, several copies of the referenced inspection
report were issued on September 16, 1993, without all the pages (i.e., the

it cannot be ascertained whether all copies were properly transmitted, we are

reissuing the entire report. The previous version of the inspection report
should be discarded.

Notice of Violation as originally issued, no further response is required.
