SUBJECT: NRC INSPECTION REPORT NOS. 50-528,529,530/93-43

f to the inspection conducted by J. Sloan, H. Freeman, and

A. MacDougall of this office, and T. Alley, of the Department of Energy, on

September 21 through November 1, 1993. The inspection included a review of

activities authorized for your Palo Verde facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The inspection identified two violations of NRC requirements, both involving

the failure to follow procedures. You are commended for identifying and

correcting the first violation, which involved performing a surveillance test

using an outdated revision. The second violation involved a maintenance

technician who inappropriately signed for a review that was required to be

signed by the ASME Section XI pump and valve engineer. These violations were

not cited because the enforcement discretion criteria in the Enforcement

Policy were satisfied.

The inspection also noted several instances in which licensee personnel took

appropriate actions in response to deficient conditions and followed plant

procedures. These included: performing a thorough follow-up to a loose jam

nut on an auxiliary feedwater pump; mounting a . camera in the refueling canal

to identify foreign objects; implementing appropriate corrective actions when

an out-of-date procedure revision was identified; removal and inspection of a

diesel generator bearing; and identification of error made during

reconstitution of a fuel assembly.

Additionally, one violation is referenced in this inspection report regarding

deficiencies you identified involving the performance of auxiliary operators.

A Notice of Violation was separately issued on October 15, 1993, and no

respcnse was requested. The violation is listed in this report for

administrative purposes only.
