NRC INSPECTION REPORT NOS. 50-528,529,530/93-40

AND MANAGEMENT MEETING
This refers to the inspection conducted by J. Sloan, H. Freeman,
A. MacDougall, B. Olson, and F. Ringwald of this office, and T. Alley, of the
Department of Energy, on August 17 through September 20, 1993, and to the
management meeting held on September 16, 1993, in the regional office. The
inspection included a review of activities authorize'd for your Palo Verde
l
facility. At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the enclosed report. The
management meeting discussed the APS Reengineering Program, regulatory
commitments, and the APS Quality Assurance Program.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
'
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violations involved:
(1) an individual who exceeded
overtime limitations without authorization, (2) the failure of operators to
follow administrative procedures regarding surveillance test results not
within acceptance criteria, and (3) inadequate instructions in a work order
for the installation of a motor-operated valve torque switch. The first two
,
violations are of concern because they involve issues which had been
highlighted in a previous NRC inspection report or your own Quality Assurance
department reports and indicate a need for closer management attention.
Both
of these violations would have been considered non-cited violations but for
the fact that your past corrective actions were not adequate to prevent
recurrence of the violations.
You are commended for your identification and correction of two other
violations, which were not cited. The first involved the failure to control
relay setting sheets for valve AFB-HV-30, and the second involved a
~
technician's failure to follow procedures for tightening the open torque
switch screw during maintenance of valve AF-35 in Unit 2.

necessary to ensure compliance with NRC regulatory requirements.

The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 19B0, Pub. L. No. 96.511.
