Conway Executive Vice President, Nuclear SUBJECT: NRC INSPECTION REPORT 50-528/93-45, 50-529/93-45, AND 50-530/93-45 This refers to the inspection conducted by Mr. D. Schaefer of this office.on October 25-29 and November 2-5, 1993. The inspection included a review of activities authorized for your Palo Verde facility. On November 5, 1993, the preliminary findings were discussed with Mr. Paul Caudill and other members of Mr. Paul Caudill
.your staff identif.ied in the enclosed inspection report.

Areas examined during this inspection included activities related to physical security as described in the enclosed inspection report. Within these areas,
.the inspection consisted of selective examination of procedures and representative records, interviews with personnel, and observations of activities in progress. The inspection findings are documented in the enclosed inspection report.
Based on the 'results of this inspection -it appeared that one of.your.
activities was not conducted in full compliance with NRC requirements as discussed in paragraph 2.c of the enclosed report. This licensee-identified violation pertains to failure of a security officer to maintain ' appropriate security compensatory measures. This violation would normally be classified as a Severity Level' IV violation. However, because you identified and took prompt corrective actions for this violation we have decided to classify this as a non-cited violation in accordance with the criteria set' forth in 10 CFR L
Part 2, Appendix C,Section VII.B(2).
The Notice of Violation from the last security insp'ection report dated' July l
29, 1993, identified your failure on nine occasions, from January through June
..
1993, to appropriately protect safeguards information. Your August 30, 1993, l
reply to this Notice indicated that following the completion of a Human Performance Evaluation System (HPES) investigation, you and your staff would

L implement corrective actions to aid APS personnel in properly securing

safeguards material. Your subsequent Safeguards Event Log dated October 29, 1993, indicated that from July through September 1993, your employees
!
discovered eight additional events involving failure to protect safeguards
!
information. These continued failures, occurring at an accelerated rate, warrant your immediate attention.
