Gentlemen:

Subject: Report of Meeting with ANPP Management

This refers to a meeting held with you, Mr. J. G. Haynes, and other

members of your staff at the ANPP Corporate Engineering Office in

Phoenix, Arizona, on February 4, 1987. The subjects discussed are

summarized in Meeting Report Nos. 50-528/87-08, 50-529/87-09, and

50-530/87-09, enclosed herewith for your information.

The discussion primarily centered on your evaluation of the causes, and

implementation of corrective actions associated with recent operational

events. Aspects of previous issues discussed at our December 17, 1986,

meeting at the NRC Region V Office were also discussed and updated.

The following points were discussed and agreements reached.

Unit 3 Diesel Generator Failure

The NRC recommended that ANPP contact additional diesel experts to

ensure that repair techniques utilized were considered sound

practice. Mr. Van Brunt, Jr. agreed to speak with other diesel

manufacturers.

Improper Documentation of Surveillances

The NRC expressed concern over instances in which plant personnel

improperly documented performance of tasks which were not actually

completed, and noted the seriousness of such problems. It was

agreed by ANPP that this subject would be discussed further, in a

future meeting, when a course of action concerning these problems

had been arrived at.

Power Supply Malfunction in PPS Caused Reactor Trip

A reactor trip which resulted, in part, from a malfunctioning power

supply in the PPS was discussed. ANPP was informed that the

responsibility for understanding complex system and component design

in detail rested with them. An effective technology transfer

program between AN.a? and their vendors was needed to ensure this

high level of understanding. ANPP concurred, stating that such a

technology transfer program was being developed.

We found our meeting to be beneficial; good communication between ANPP

and the NRC is fundamental to our understanding of how you approach and

resolve plant problems. We look forward to discussing the effectiveness

of your corrective actions during a future meeting.
