Investigation revealed that the instrument had been inoperable since a design change was made on July 15, 1986 and retesting had not yet been performed. This period of time exceeds the Technical Specification Action Statement allowable time of 30 days. The root cause of this event was personnel error on the part of the Outage Planning Department and the Operations Department. Corrective action consisted of initiation of repair and retesting of the instrument. In addition, Planning and Operations Department personnel will be counseled with regard to Technical Specification operability requirements. This Special Report is being submitted pursuant to Technical Specification 3.3.7.2.

On October 3, 1986, it was discovered that one of the seismic !
Monitoring Instruments was inoperable. An investigation revealed I that this instrument had been inoperable since a design change was made on July 15, 1986. In addition, retesting following this I modification was deferred and had not yet been performed. The period of time of inoperability exceeded the Technical Specification Action Statement allowable time of 30 days. Upon this discovery, action was initiated to restore the instrument to operable status and an investigation into this event was undertaken.
hBbLYB1B DE DCCDEBEBCE On March 20, 1986, a Design Change Package (DCP) was assembled and approved which installed lightning arrestors on the Free Field 1
Triaxial Time-History Accelerometer located 60 feet below grade.
This DCP was then forwarded to the Planning Department to schedule the actual work. During preparation of the work package, Planning failed to recognize that implementation of this DCP and associated retesting would render the channel inoperable. This error resulted in failure to specify on the Work Order that a Technical Specification Action Statement applied. The actual modification was completed on July 15, 1986 with retesting to be performed on a different Work Order. When permission was granted by the Operations Department to start the job, the Technical Specification operability. requirements were also overlooked and the Action Statement was not entered. The Work Order which was to perform the retesting was subsequently cancelled by the Planning Department and the required retesting was deferred to the next scheduled I&C Functional Test. This decision to defer retesting resulted in another error in that the next scheduled Functional Test was not

Immediately upon discovery, action was taken to initiate repair and retesting of the Seismic Monitoring Instrument. The individuals involved will be counseled with regard to the ramifications of this event by the respective department management. In addition, this event will be reviewed with all Planning and Operations personnel that are responsible for determining Technical Specification operability requirements.
