' DOCKET NO. 50-354 This Special Report is being submitted pursuant to the requirements of Hope Creek Technical Specification 4.8.1.1.3, due to the "C" Emergency Diesel Generator (EDG) failures on December 6 and 7, 1995.
On December 6, 1995, the EDG was started to perform a monthly surveillance test.
Approximately five minutes into the test, while the EDG was running unloaded, technicians noticed that a recorder used for the test was in the wrong mode and requested that the EDG be shutdown so the data could be recollected.
When the operator attempted to secure the EDG, he noticed that the stop indication was illuminated, but all other indications showed that the EDG was still running (i.e.,
- voltage, frequency, field current, etc.).
Subsequently, the EDG was manually shutdown.
The initial cause of the failure was determined to be a faulty speed switch, which controls vital functions of the EDG (i.e.,
maintain jacket water temperature, enables cooling of the EDG, etc.).
This condition would have ultimately resulted in a failure had the EDG been subsequently loaded.
On December 7th, the speed switch was replaced and monthly surveillance test was run again.
The "C" EDG again failed in the same manner as it had on December 6th.
As a result of further investigation, it was determined that the cause of both of the "C" EDG failures was due to a faulty tachometer relay signal generator, which provides input signals to the speed switch.
On December 8th, the tachometer speed signal generator was replaced.
On December 9, 1995, the "C" EDG retested satisfactorily and was declared operable.

These were the first and second failures of the "C" EDG in the last 100 valid tests.
The current test frequency for the "C" EDG is weekly, in accordance with the guidance provided in Technical Specification Table 4.8.1.1.2-1, which
$[1 requires weekly testing when there are two or more fajlures in the last 20 valid tests.
