ventilation system (FRVS) high range noble gas monitor being inoperable for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

On September 13, 1995, during performance of a routine functional test of the FRVS vent radiation monitor, a blown power fuse was found.

This blown poder fuse rendered the controller for the sample flow through the FRVS radiation monitoring system skid inoperable, including the high range noble gas monitor.

Initial corrective actions taken to prevent recurrence of the blown power fuse included the replacement of a transformer in the flow controller power supply.

On September 26, 1995, the transformer replacement was completed for the FRVS radiation monitoring system skid, however, the power fuse was blown again during retesting.

Subsequently, the entire flow controller card was removed and shipped to the manufacturer to determine the cause of 4

the blown power fuses and to make appropriate repairs.

The card was an obsolete part and no other cards were available to replace the one sent to the manufacturer.

The manufacturer concluded that power transformer for the card needed to be replaced.

The card was repaired and calibrated by the manufacturer and returned.

The repaired card was re-installed on October 23, 1995, and upon completion of required testing, the FRVS high range noble gas monitor will be restored to an operable status.

As specified in Technical Specification Table 3.3.7.5-1, Action 81 b, this special report was required to be submitted to the NRC i

within 14 days following the event.

However, it was not i

discovered until 10/10/95 that this special report was required by the Technical Specifications.

The cause of this late special report will be addressed in a subsequent Licensee Event Report.
