Dear Sir:
Hope Creek Generating Station Docket No. 50-354 Unit No. 1 Special Report 91-03-01 (Supplement) l This supplement to Special Report 91-03-00 is being submitted as per our commitment contained in the original report.
On May 22, 1991 at 2117 hrs, during performance of a monthly Surveillance operability run, the "D" Emergency Diesel Ger.erator failed to start when a manual start signal was initiated from the Main Control Room.
Initial troubleshooting could not determine the cause for the start failure and a second attempt was made to complete the surveillance.
At 2137 hrs. the test was rerun and the D EDG started successfully as required.
The monthly surveillance

was completed satisfactorily and the EDG-was placed in standby.
The probable cause for a failure of this type is a lack of fuel boost when the engine start signal is received.
This condition can be attributed to either mechanical failure or i
mispositioning of the Min / Max Fuel position switch on the governor.
A check of the mechanical components associated with the fuel boost system was conducted by System l
Engineering working in close liaison with Colt Pielstick l
field service management.
No mechanical failures were noted.
L Operations Department personnel who performed the test have L
stated that no repositioning of the Min / Max Fuel Boost position switch was made between test runs.

Since May 22, 1991, the D EDG has been started and run successfully on eleven different occasions with no repeat of the failure.
As the conditions which led to the failed start l
cannot be replicated or precisely identified the cause of the
[
failure is indeterminate.
h To further reduce the possibility of a repeat occurrence, l
Plant Operating Procedures will be modified to verify proper Min / Max Fuel Boost Position Switch prior to each surveillance run.
