SPECIAL REPORT 96-01 HOPE CREEK GENERATING STATION FACILITY OPERATING LICENSE NPF-57 DOCKET NO. 50-354 This Special Report is being submitted pursuant to the requirements of Hope Creek Technical Specification 3.3.7.5-1, Action 81 b, due to the South Plant Vent High Range Noble Gas Monitor being inoperable for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.
On July 2,1996, Technical Specification Action Statements 3.3.7.5," Accident Monitoring Instrumentation", and 3.3.7.11, " Radioactive Gaseous Efiluent Monitoring Instrumentation", were entered to support troubleshooting an upward trend in the South Plant Vent Radiation Monitoring System flow. Upon the system being declared inoperable, the Radiation Protection department stopped the radioactive gaseous eflluent monitoring sample pump, which eliminated automatic start capability of the accident monitoring sample pump and rendered it inoperable. Compliance with required compensatory actions was met.
Troubleshooting on July 2,1996 identified the flow instrument loop calibration out of specift tion. Further troubleshooting found the square root extractor in the instrument loop out of calibration. The air supply tubing, air filter / regulator assembly, and square root extractor were replaced by July 11,1996 and the instrument loop was recalibrated.
On July 12,1996, the flow instrument loop was again determined to be out of calibration.
Further troubleshooting identified that the flow transmitter and the pneumatic to current converter were out of calibration. On July 15,1996, the flow transmitter and pneumatic to current converter were replaced.
The flow instrument loop will be bench tested with the new flow transmitter and pneumatic to current converter. The South Plant Vent Radiation Monitoring System will be returned to operable status upon completion of trouble shooting and testing. The final cause and any additional corrective actions will be documented in the Corrective Action Program.
