Il0PE CREEK GENERATING STATION DOCKET No. 50-354 UNIT NO. 1 SPECIAL REPORT 91-03-00 Thin Special l'eport in nubmitted purnuant to the requirements of Ilope Creek Technical Specification 4.8.1.1.3 and Regulatory Guide 1.108, Revision 1 due to the teot failure of the "D"

Emergency Dieuel Generator (EDG).

Theon documento require a written report to the NRC within 30 dayn of a valid or non-valid start failure.

On 5/22/91 at 2117, during portormance of a monthly operability surveillance rua. the "D" EDG failed to start uhen a manual start was initiated from the control room.

Prior t o tripping, the engine rolled for approximatoly 15 seconds but did not start, with indicated generator voltage increasing to about 200 volto and frequene" at 56 hertz.

An immediate debriefing of equipment operatorn (E0's, nor-licensed) stationed at the local control panelo did not reveal any abnormal indications or equipment malfunctionn when the start attempt was initiated.

Initial troubleshooting could not determinn a reason for this failure to start, and a second attempt was made to complete this evolution at 2137.

The "D"

LDG started as required, and monthly surveillance was completed in a satiofactory manner.

Followup investigation by Syntema Engineering and Maintenance dept twenta determined that the engine rolled, but did not start, and failed to reach 125 RPM due to a lack of fuel boost.

The reason for the lack of fuel boont han not been determined at thin time, and investigation / troubleshooting into the i

event is continuing.

The curveillance procedure was reviewed for adequacy, and was deternined to be satisfactory.

The current investigation is focusing on equipment malfunctions and personnel errors that could result in an EDG failure to start, given the indications observed and a review of post-event documentation.

Tenting intervals for the "D'

EDG have been increased to once per 7 days.

Technical Specifications require testing frequency to be increaned to once per week if the nuinber of start failures exceeda 2 out of the last 20 or 5 out of the ' ant 100 attemptn.

On 1/7/91, the "D"

EDG failed to start during the perf orinance of an 18 month integrated test via an Emergency Core Cooling Syntem (ECCS) actuation tent signal.

Since 5/22/91, the "D"

EDG han been ntarted and run succeusfully 5 timen.

A supplement to thic report in expected to be submitted by 8/30/91 detailing the results of our continuing inventigation.
