n.., n ei The North Plant Vent (NPV) Radiation Monitoring detector (lSP-RE-4873B) was declared inoperable on 2/4/87 at 0920 due to a damaged aluminum foil window.
The detector could not be made operable in 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, thus the requirements of Technical Specification 3.3.7.5, Action 81 applied.
Those actions included initiating a pre-planned alternate method of sampling NPV effluents (12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> grab samples) and submitting a Special Report within 14 days.
The NPV Radiation Monitoring Detector was repaired and declared operable on 2/17/86 at 1640.
This Special Report is being submitted pursuant to Technical Specification 3.3.7.5.

1640 DESCRIPTION Of OCCURRENCE On February 4, 1987 at 0920, the NPV Low Range Noble Gas Detector was declared inoperable after I&C Department discovered, during performance of a
calibration surveillance, that the detector aluminum foil window was damaged.
The cause of the damage could not be determined.
The mid/high range detectors automatically start as the low range detector reaches its upper limit.
- Thus, with the low range detector inoperable, the entire skid becomes inoperable because the automatic "upscaling" feature of the detector skid is disabled.
As a result of this determination the entire NPV Radiation Monitoring Skid was declared inopetable at 1630.
The monitoring skid was returned to operability on 2/17/87.
ANALYSIS QE OCCURRENCE Technical Specification 3.3.7.5, Action 81 specifies that:
ACTION 81 -
With the number of OPERABLE accident nonitoring instrumentation channels less than required by the Minimum Channels OPERABLE requirement, either restore the inoperable channel (s) to OPERABLE status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, or:
a.
Initiate the preplanned alternate method of monitoring the appropriate parameter (s), and b.
Prepare and submit a
Special Report to the Commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action
- taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status.

On 2/6/87, it was determined that the NPV radiation monitoring skid would not be returned to an operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.
As a result, the preplanned alternate method of monitoring' noble gas activity was initiated.
This method consisted of:

Taking 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> grab samples for analysis of low range activity.
2.
Direction to manually initiate the mid and high range detectors if indicated by the results of the 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> grab samples.
The NPV radiation monitoring skid was returned to operability on 2/17/87.
This Special Report is being submitted pursuant to Technical Specification 3.3.7.5, Action 81 b.
CORRECTIVE ACTION A
work order was written to replace the aluminum foil window on the low range detector.
The window was replaced and the monitor returned operability on 2/17/87.
