E NIAGARA M0 HAWK POWER CORPORATION NINE MILE POINT UNIT 2 DOCKET N0. 560-410 Final Report for a Problem Concerning Damage to Insulation of Rockbestos Cable (55(e) 19)

Description of the Problem-The problem concerns Class lE multi-conductor cable supplied by the Rockbestos Company. . The manufacturer reported the problem to the Nuclear Regulatory Commission under 10CFR21, via letter dated May 11, 1984 (copy enclosed). As stated in this report the damage to the cable insulation was caused during the process of reworking outer jackets and the damage was confined to work performed by one operator. The vendor has identified 12 reels, supplied to Nine Mile Point Unit 2, tnat may have the damaged cable segments.

Analysis of Safety Implications The vendor has indicated in its 10CFR21 notification, that there is a possibility of failure of the cable in question to operate in an accident environment. Tne failure of a Class IE cable could result in the loss of a safety related circuit. Therefore, if this problem were to have remained uncorrected, it could have adversely affected _the safety of operations of the plant.

Corrective Action

1. Cable from eight suspect reels has not been used. These reels will be returned to the Seller.
2. The installed suspect cable with jacket rework will be removed from safety related circuits.

These corrective actions will be completed by October 25, 1984. y L ,
