. APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. CPPR-120

As a result of the inspection conducted on July 8 - August 11, 1985, and in

, accordance with NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

10 CFR 50 Appendix B Criterion XI, Test Control requires a testing program to assure all testing, including preoperational tests, required to demon-strate that structures, systems, and components will perform satisfactorily is identified and performed in accordance with written test procedures

, which incorporate the requirements and acceptance limits of design docu-i ment Listed below are three examples of failure to satisfy this require-men , FSAR Section 14.2.12.1.48, Fuel Handling and Vessel Servicing Equip-ment, and the GE Preoperational Test Specification for Hope Creek (No-A4103300-32(1)-3: 22A2271AZ, Revision 2) state that all inter-locks associated with the refueling and service platforms will be verified and included as acceptance criteria in the preoperational

test (PTP).

Contrary to the above, appr oved PTP KE-1, Fuel Handling and Vessel Servicing Equipment, Revision 0, did not include steps to properly verify a refueling interlock that prevents moving the refueling plat-form over the core in the Startup mode with all rods in and all hoists unloade . FSAR Section 14.2.12.1.54, Remote Shutdown Panel, states that proper function of control room alarms associated with transfer of control <

of individual components to the remote shutdown panel will be included as acceptance limits for the PT Contrary to the above, proper function of alarms was not included as an acceptance limit for PTP SV-1, Revision 0, Remote Shutdown Pane . FSAR Section 14.2.12.1.54, Amendment 11, Remote Shutdown Panel, states that all valves and controls associated with the remote shutdown panel will be tested and included as test acceptance criteri Contrary to the above valve IEG-HV-2520B, SACS cooling inlet to RHR pump B seals and motor bearings, was not included for testing in PTP SV-1, Revision 0, Remote Shutdown Pane This is a Severity Level IV violation (Supplement II).
