APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-57 As a result of the inspection conducted on October 14 - November 17, 1986, and in accordance with " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy 1986),
the following violation was identified:
A.
Technical Specification 6.8.1 requires that written procedures be estab-lished, implemented, and maintained covering the activities recommendeo in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978, wFich includes Administrative procedures.
Operations Department Administrative Procedure OP-AP.ZZ-108(Q), " Removal and Return of Equipment to Service," requires that all applicable Technical Specification Limiting Conditions for Operations be reviewed to determine if equipment status requires entry into an action statement.
Contrary to the above, on October 22, 1986, core spray pressure trans-mitter E-21-N055H was found to be isolated and the applicable Technical Specification 3.3.3 action statement was not entered.
This is a Severity Level IV violation (Supplement 1).
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further l
violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
