+n ENCLOSURE i NOTICE OF VIOLATION 'l Public Service Electric ' Docket Nos. 50-272, 50-311, i and Gas Company' and 50-354 Salem Nuclear Generating - License Nos. DPR-70, DPR-75, Station,' Units 1 & 2 and NPF-57 o Hope Creek Generating Station During an NRC inspection' conducted between June 14 and July 25,1992, violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedures for .NRC Enforcement Actions," the violations are listed below:  ;

10 CFR 50.9, Completeness and accuracy of information, requires, in part, that 4

-information required by statute or by Commission regulations, orders, or license conditions to be maintained by the licensee shall be complete and accurate in all material respects.

Hope Creek Technical Specification (TS) 6.8.1 requires that written procedures shall be established, implemented, and maintained covering the activities referenced in. the PSE&G Fire Protection Program. Nuclear Department Site Protection Procedure M10-FDD-004, Rev. 3, " Action For Inoperable Fire Protection Systems, Hope Creek Generating Station (HCGS)," written to satisfy the requirements of TS 6.8.1, requires, in Step 6.2, that certain compensatory measures, including the establishment of hourly or continuous fire watches, be taken when a fire protection system or component at HCGS is determined to be inoperable.  !

Salem Units 1 and 2 TS 3.7.ll.a require that the licensee, with one or more of the required fire barrier penetrations non-functional, within one hour either establish a continuous fire _ watch on at least one side of the affected penetration, or verify the operability of fire detectors on at least one side of the non-functional fire barrier and establish an hourly fire watch patrol.

Contrary to the above, the licensee determined several instances in which personnel assigned to fire watch patrols failed to perform or complete the function as required by the technical specifications, and falsified records pertaining to the performance of fire watch activities. Specifically, during the period of February - June 1992, the records of i 19 of 35 fire watch personnel reviewed were identified as being incomplete or inaccurate l in that the records indicated that certain fire watch patrols were completed when the {

patrol was actually incomplete. These falsified records were material in that they l evidenced that checks had been made when they had not.
