APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 354 Hope Creek Generating Station License No. NPF-57 As a result of the inspection conducted on July 12, 1988 - August 29, 1988, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR 2, Appendix C (Enforcement Policy 1987), the following violation was identified:

A. 10 CFR 50, Appendix B, Criterior XIV specifies that measures shall be established for indicating the operating status of components of a nuclear power plant, such as tagging valves and switches, to prevent inadvertent operation.

Contrary to the above, on August 17, 1988, the measures established for tagging valves were not adequate, in that the relay for valve GS-HV-4958 was erroneously removed on May 27, 1988, rendering the valve inoperable; and the valve was not restored to operability on May 30, when the deficient system tagging was removed.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Public Servfce Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
