Appendix NOTICE OF VIOLATION The Cleveland Electric Illuminating Docket No. 50-440 Company As a result of the inspection conducted on July 8 through August 5,1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violation was identified:

10 CFR 50, Appendix B, Criterion 6, " Document Control," as implemented by the applicant's Quality Assurance Plan, Section 6, Revision 1, dated May 6, 1985, requires that the issuance of instructions shall be controlled including approval for release and distribution to and use at the location where the activity is accomplishe Contrary to the above, Temporary Change Notice (TCN)-005 for Plant Administrative Procsdure (PAP)-1402, Revision 2, " Control of Lifted Leads, Jumpers, Temporary Electrical Devices and Mechanical Foreign Items," which modified the procedure for controlling temporary alterations affecting equipment under test jurisdiction and temporary alterations affecting control room annunciators, was assigned an effective date the same as the signature approval date and no special provisions were made for expedited distribution or training to the new program requirements. Additionally, the procedure change was not entered into the operations manual in the control room until seven days after the effective date of the procedur This is a Severity Level V violation (Supplement II).

Pursuent to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for the violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violation; and (3) the date when full compliance will be achieve Consideration may be given to extending your response time for good cause show 'AUG 2 61985 Dated RFM a R.
