Public Service Electric and Gas Company 3 APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-50 As a result of the inspection conducted on March 17 - April 30, 1986, and in accordance with NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

A. Technical Specification 3.6.5.3 states that five Filtration, Recirculation and Ventilation System (FRVS) recirculation units and two ventilation units shall be operable during core alterations.

Contrary to the above on April 16, 1986, all six FRVS recirculation units and both FRVS ventilation units were inoperable during core alterations, in that the associated controls were placed in lock-out; disabling all system automatic start functions.

This is a Severity Level IV violation (Supplement 1).

B. Technical Specification 3.1.5 requires the standby liquid control system to be operable when any control rod is withdrawn. Section 4.1.5 of Tech-nical Specification requires the standby liquid control system to be demonstrated operable at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> by verifying that the available volume of sodium pentaborate solution is within the limits of figure 3.1.5-1.

Contrary to the above on April 22 and April 23, 1986, control rods were withdrawn during fuel load operations and the available volume of sodium pentaborate solution was not verified to be within the limits of figure 3.1.5-1.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation

in reply, including
(1) the corrective steps which have been taken the the l

results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good l

cause is shown, consideration will be given to extending this response time.
