APPENDIX A NOTICE OF VIOLATION Public Service Electric & Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-57 During an NRC inspection conducted on September 24 -

October 3, 1986 a violation of NRC requirements was identified. The violation involved improper torque setting on select Rosemount transmitters which affected their environ-mental qualification. In accordance with the General Statement of Policy and Procedure for NRC enforcements actions 10 CFR Part 2 Appendix C (Enforcement Policy 1986) the violation is listed below:

10 CFR 50 Appendix B, Criterion V, states, in part, " Activities affecting quality shall be prescribed by documented instructions and shall be accomplished in accordance with these instructions."

L Licensee procedure IC-GP.ZZ-053(Q) - Rev. 3, General Work Procedure -

Maintenance of Environmentally Qualified Rosemount Transmitters, states in section 5.2.36 "using the Rosemount torque adaptor, torque cover to 200

(+ or - 10) in.-lbs."

Contrary to the above, on September 30, 1986, the covers for Rosemount transmitters 1GSPT-4960A3 and IBPT-N050A-C71 were not torqued to 200 inch-lbs. causing these instruments to be potentially exposed to different environmental conditions than those for which they were qualified.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Hope Creek Generating Station is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved; (3) corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
