APPENDIX A NOTICE OF VIOLATION Public Service Electric & Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-50 During an NRC inspection conducted on June 23 - July 3, 1986, a violation of NRC requirements was identified. The violation involved the failure to perform the startup tests for full core shutdown margin and source range monitor response to rod withdrawal in conformance with approved procedures.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C, the violation is listed below.

10 CFR 50 Appendix B, Criterion XI, requires in part that, "a test program shall be performed in accordance with written test procedures."

With regard to the above described testing, Technical Specification 6.8.1 stated, similarly, that written procedures shall be established, implemented and maintained.

Contrary to the above, on June 28, 1986 during the performance of startup test procedures TE-SU.ZZ-041, " Full Core Shutdown Margin Demonstration,"

and TE-SU.SE-062, " Source Range Monitor Response to Rod Withdrawal," test personnel failed to record SRM readings, update the Inverse Multiplication Data Sheet, plot the results and make a 1/M Criticality Prediction as required by step 5.2.1 of TE-SU.ZZ-041 and step 5.2.1 of TE-SU.SE-062 when control rod 18-39 reached the notch position identified in the rod pull listing.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Hope Creek Generating Station is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
