APPENDIX A
.l NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354-
' Hope Creek Unit 1 Nuclear Plant License No. NPF-57 As a result of the equipment qualification inspection of January 26-29, 1988 and in accordance with NRC Enforcement Policy (10 CFR 2, Appendix C) the following violation has been identified:
10 CFR 50, Appendix B, Criterion V states, in part, that: "Activities affecting quality shall be accomplished in accordance with instructions, procedures..."
i Procedure number VPN-NSP-07, Section 8.2.1 states, in part, that:
"The use of whiteout and/or obliteration is not permitted on Quality Assurance records. Changes shall be made by a single line drawn through the item with new information written in, initialed and dated."
Contrary to the above, on January 29, EQ file numbers J-601, J-313 and E-171 contained deletions, whiteout changes of the original text that had not been initialed or dated.
This is a Severity Level V violation.

explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is_shown, consideration will be given to extending this response time.
