APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-57 As a result of the inspection conducted on February 9, 1988 - March 21, 1988, and in accordance with "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy 1986), the following violation was identified:

Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained covering the activities recommended in Appendix A of Regulatory. Guide 1.33, Revision 2, February 1978, which includes administrative procedures.
I&C Calibration Procedure IC-DC.ZZ-057, "Device / Equipment Calibration-Dwyer Differential Pressure Switch Series 1800 and 1900" specifies correct system restoration including an independent verification of system lineup at the conclusion of the device calibration.
Contrary to the above, when on February 26, 1988, procedure IC-DC.ZZ-057 was implemented, a "B" channel filter, recirculation and ventilation system fan flow pressure switch was not correctly restored to service.
Additionally, the independent verification of system lineup failed to identify the discrepancy.

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results a;hieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
