Appendix A Notice of Violation Public Service Electric Docket No. 50-354 and Gas Company License No. NPF-50 Hope Creek Generating Station As a result of the inspection conducted on December 3-5, 1986, and in accord-ance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

Technical Specification 6.11, " Radiation Protection Program," requires, in part, that procedures for personnel radiation protection be prepared and adhered to for all operations involving personnel radiation exposure.

1. Procedure RP-AP.ZZ-1010, Revision 2, requires, in part, in section 4.4 that personnel performing a job under a radiation work permit (RWP) sign the RWP sign in sheet prior to initial entry to indicate that the RWP has been read and understood and will be complied with.

Contrary to the above, on or about November 23, 1986, at least eight individuals did not sign the sign in sheet for Revision 2 of RWP No.

. 86-OH-599-2, Disassembly and Repair of RHR Pump B, to indicate that they read, understood and would comply with its provisions prior to their initial entry.

2. Procedure RD-AP.ZZ-101Q, Revision 2 requires, in part, in section 5.2.2.16 that copies of radiation work permits be ~ included in work packages and posted at the work location.

Contrary to the above, on December 4,1986 a copy of RWP No.

86-OH-651-1, Disassemble / Inspect Unit 1 RHR Pump, was not included in the work package for this activity or posted at the work location.

This is a Severity Level IV violation. (Supplement IV)

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

, Where good cause is shown, consideration will be given to extending this response time.
