• APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-50 As a result of the inspection conducted on July 7-24, 1986, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Ac-tions," 10 CFR 2, Appendix C (Enforcement Policy 1986), the following viola-tions were identified:

A. 10 CFR 50, Appendix B, Criterion V requires, in part, that " Activities affecting quality shall be prescribed by documented procedures appropriate to the circumstances." Technical specification 6.8.1.a through its reference to Regulatory Guide 1.33 Revision 2, 1978 and ANSI N18.7-1976 Section 5.3 contain similiar requirements.

Con' sry to the above, on July 23, 1986, the inspector noted, prior to imp! smentation, that step 5.3.1 of power ascension test TE-SU.BD-142 was not appropriate to the circumstances because it would have resulted in the termination of reactor core isolation cooling flow to the reactor vessel.

This is a Severity Level V violation (Supplement I).

B. 10 CFR 50, Appendix B, Criterion XI, requires, in part, that " Testing be performed in accordance with written test procedures," and further re- t; quires that " Test results shall be evaluated to assure that test require-ments have been satisfied." ]b i

Contrary to the above, during the performance of power ascension test TE-SU.ZZ-041 on June 29, 1986, the calculations required by step 2.15 were not performed in accordance with the equations provided in the procedure.

Also, your review of the test results completed on July 17, 1986 failed to identify this error.

This is a Severity Level V violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or expla-nation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
