Appendix NOTICE OF VIOLATION Illinois Power Company
. Docket No. 50-461

As a result of the inspection conducted on July 8 though August 20, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violation was identified:
10 CFR 50, Appendix b, Criterion V, as implemented by Illinois Power Startup (IPSU) administrative procedure SAP-11 states that activities affecting ouality shall be prescribed by and accomplished in accordance with procedures of a type appropriate to the circumstances.
a.
Contrary to the above, preoperational test procedure PTP-RI-01,
" Reactor Core Isolation Cooling," was not of a type appropriate to the circumstances in that the procedure did not control the sequence of performing the test.
b.
Contrary to the above, preoperational test procedure PTP-FC/SM-01,
" Fuel Pool Cooling and Cleanup / Suppression Pool Makeep," was not accomplished in accordance with the procedure in that the sequence of performing the test was not controlled.
This is a Severity Level V violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when' full compliance will be' achieved.
Consideration may be given to extending your response time for good cause shown.
