AECM-85/0242 Page 1 of 3 NRC VIOLATION 50-416/85-20-01 Notice of Violation 10CFR 50, Appendix B, Criterion V states activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Contrary to the above, certain procedures were inadequate in that:

a. Maintenance Procedure 07-S-53-C34-4 required placing the reactor feed pump controls in manual and lifting a steam flow input lead to the reactor vessel level system which resulted in a high reactor vessel level scram.

b. Plant Administrative Procedure 01-S-06-24 did not cicarly specify that a safety evaluation form (Attachment I of 01-S-06-24) was required to be filled out for temporary change No. 8 to 07-S-53-C34-4. This temporary change modified the facility as described in the FSAR.

I. ADMISSION OR DENIAL OF THE ALLEGED VIOLATION Mississippi Power & Light Company (MP&L) admits to the alleged violation. The violation did not affect the health and safety of the public.

II. REASON FOR THE VIOLATION IF ADMITTED MP&L's review of these violations revealed the following:

a. As a prerequisite to Startup Test SU-25, the I&C Section was requested to perform a calibration on all main steam line flow transmitters. Maintenance Procedure 07-S-53-C34-4 provided the instructions for calibration of these instruments. When the I&C technicians began their calibration, it was noted that the reactor level began oscillating even though reactor level was in single element control It was determined that this was being caused by the steam program function generator, NIC341K652, which adds a 0-5 inch level program signal to the control circuit and varies with total steam flow. To stabilize the oscillations in reactor level, a change in the maintenance procedure was required.

A Temporary Change Notice (TCN) was written against the procedure allowing a dummy signal to be input to steam flow / level summer K651. This was donc so that the calibration of summer K652 could continue as already required by the procedure. During the engineering evaluation of the TCN it was mistakenly determined that the signal output of K651 would drop the equivalent of 15-20 inches in Icvel. However, when the lead to K651 was lifted, instead of a ~

15-20 inch drop as anticipated, the steam flow / level summer saturated low. The reactor recirculation pumps transferred to the low frequency motor generators because of an indicated low reactor level. This caused actual Icvel to swell resulting in a high level scram.

b. The guidance in Plant Administrative Procedure 01-S-06-24, " Safety and Environmental Evaluations", did not adequately direct that a safety evaluation be performed if the screening evaluation identified the change as a change to the facility as described in the Final Safety Analysis Report (FSAR).

III. CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED

a. The temporary change notice was cancelled and the procedure was successfully completed while the plant was still shut down.
b. Plant Administrative Procedure 01-S-06-24, " Safety and Environmental Evaluations", was revised. The revised procedure provides the safety evaluation screener with direction that a safety evaluation is required if the review determines that the change is a change to the facility as described in the FSAR.

IV. CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS MP&L considers the actions discussed in III above sufficient to prevent further violations.

V. DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED Full compliance has been achieved.

Contrary to the above, on June 6, 1985, as the result of a steam leak in the steam tunnel, Valves QlElF207 and QlE51F208 required by the Local Leak Rate Test Valve Lineup Procedure 09-S-08-2 to be closed were found in the open position.

I. ADMISSION OR DENIAL OF Ti!E ALLEGED VIOLATION Mississippi Power & Light Company (MP&L) admits to the alleged violation. The violation did not affect the health and safety of the public.

II. REASON FOR THE VIOLATION IF ADMITTED The verification technique used by two operations personnel resulted in valves being inadequately verified closed. The operators reached across pipes to tap on the T-handled valves from a distance rather than stand over the valves and verify their position properly.

III. CORRECTIVE STEPS WilICil IIAVE BEEN TAKEN AND Tile RESULTS ACIIIEVED A) The operators involved were counseled on the proper techniques of valve position verification; the importance of proper independent verification; and the significance of signing a valve lineup certifying valve positions.

B) The Manager Plant Operations, Operations Superintendent, and the operators involved in this incident met with each of the operating shifts. During these meetings, the particulars of this incident, significance of signatures on lineups, and the importance of proper valve position verification were discussed with the operators.

C) To verify that no other valves had been improperly positioned, systems in the steam tunnel were walked down and valve positions verified.

IV. CORRECTIVE STEPS WHICII WILL BE TAKEN TO AVOID FURTilER VIOLATIONS MP&L considers the actions discussed in III above sufficient to prevent further violations.

V. DATE WilEN FULL COMPLIANCE WILL BE ACilIEVED Full compliance has been achieved.
