VRAIN NUCLEAR GENERATING STATION PUBLIC SERVICE COMPANY OF COLORADO 16805 WELD COUNTY ROAD 19 1/2 PLATTEVILLE, COLCRADO. 30651-9293 REPORT NO. 50-267/S2-017/03-L-0 Final IDENTIFICATION OF OCCURRENCE: Ouring the pericd May 12, 1982, to May 25, 1982,-with the reactor at power operation, the plant was operated in a degraded mode of LC0 ?.2.10 on two separate occasions. These events are reportable per Fort St. Vrain Technical Specification AC 7.5.2(b)2. EVENT OESCRIPITCN: Event #1 At 1713 hours on May. 12, 1982, an increase.in reactor power from 40% power caused the average core outlet temperature to increase above 1200 F. Primary coolant chemistry results indicated ~ 0 ppm' CO, 18.0 ppm CO 2 , and 4.6 ppm H2 0, equaling 22.6-ppm total oxidants. This degraded made of LCO 4.2.10 continued until _1630 hours, May 13, 1982, when reactor power was reduced, and the . average core outlet temperature decreased be"ow the 1200 F limit of LCO 4.2.10. (See Table 1) Event #2 An increase in reactor power from 50% power caused the average core outlet temperature to' increase above 1200*F with total primary coolant oxidants - greater than 10 ppm. -This degraded mode of LCO 4.2.10 commenced at 0323 hours on May 19, 1982, and continued until 0820 hours on *.ay 25, 1982, when total primary coolant _ oxidants were reduced below the 10 ppm limit. During this period, the maximum level of total oxidants reached was approximately 73.3 ppm at 0400 hours on May 20, 1982. (See Table 1)-

REPORTABLE OCCURRENCE S2-017 ISSUE O page 2 of 5 CAUSE DESCRIPTION: The reactor -plant had been in a shutdown or low power operating condition over a five month period for a major system modification outage. During this period, the prestressed concrete reactor vessel had been at subatmospheric pressure several times for various activites. These conditions allowed for a gradual buildup of oxidants witnin the primary coolant and adsorption into the core graphite. Subsequent operation caused-off gassing of the moisture and other oxidants from the core graphite and insulating boundaries, which resulted in the two events. CORRECTIVE ACTION: Event #1 A reduction of reactor power and temperature made LCO 4.2.10 no longer applicable. The average core outlet temperature was decreased below 1200 F at 1630. hours on May 13, 1982. Event #2 Normal operation of the primary coolant purification system was_used to reduce total primary coolant oxidants below 10 ppm. At 0820 hours on May 25, 1992, total oxidants equalled 8.5 ppm.
