Results Of the areas inspected, one item of noncompliance was found (Infraction - Sub-standard fire damper installation - Paragraph 5.a(6)); and one deviation was found (Deviation - Power supply for electric fire pump not designed for locked rotor current paragraph 5.r.(1)).

Exit Interview The inspection scope and findings were summarized on October 31, 1980 with those persons indicated in Paragraph I above.

I.icensee Action on Previous Inspection Findings a.

(Close) Deviation Item (416, 417/79-10-01):

Substandard fire pump suction piping. The eccentric tapered reducer in the suction piping to the west fire pump has been property aligned. This item is closed.

(Close) Deviation Item (416, 417/79-10-02): Unapproved fire doors in fire barriers of fire pump house.

Approved fire doors have been installed. This item is closed.

c.

(Closed) Deviation Item 416, 417/79-10-03):

Substandard sprinkler installation for fire pump house and diesel generator buildings.

Automatic sprinkler protection has been provided for all portions of the diesel fire pump rooms. Protection is provided directly over the sprinkler riser in the electric fire pump room which should prevent fire damage to this protion of the sprinkler system, sprinkler protec-tion has also been extended to the area beneath the mezzanine. Due to these modifications this deviation item is closed; however, the sprinkler system in the diesel generator building contains several deficiencies which have been identified by the licensee and are apparently to be corrected. Two major discrepancies are the lack of air supervision for the pre-action sprinkler piping and the sprinkler piping does not appear to be seismic supported as required by the construction specifications.

These are identified as an Inspector Followup Item (416/80-25-04 and 417/80-15-04), Sprinkler system discrepancies - diesel generator building, and will be reviewed during a subsequent NRC inspection.

(Closed) Unresolved Item (416/80-10-04):

Evaluation of manual sprinkler system for diesel generator building. The licensee is to arrange the sprinkler systems in this building to be automatically operated by means of a flame detection system. However, this item was apparently not a requirement and is not an enforcement item. This item is closed.

(Clos *d) Deviation Item 416/79-10-06: Smoke detection system install-e.

e ation deficiencies. The licensee has developed Technical Specifica-tion Nos. 9645-M-651.1, Installation of wall and ceiling - Mounted Smoke Detectors, to assure proper installation of the smoke detection systems. Also, the licensee is to revise the Appendix 9A of the FSAR

-3-to indicate the areas which are to be provided with area fire detection and tb portions of the plant which are to only be provided with spot detection.

The fire and smoke detection installations will be reviewed during a future NRC inspection, but for the present this item is considered closed.

f.

(Open) Unresolved Item 416/79-10-06 and 417/79-10-04): Fire Rating of-doors between turbine :.nd control buildings. These doors are heavy weight blast type vault doors. A review of the procurement documents indicates that these doors have not been subjected to a fire test by a recognized fire testing laboratory but are cettified by the manufacturer to be equivalent to fire rated doors. This item is to be evaluated during NRC/NRR fire protection visit scheduled for the week of November 17-21, 1980. Therefore this item will remain open pending the results of this inspection.

(Closed) Unresolved Item (416/79-10-07 and 417/79-10-05):

QC (" Construction") records storage facility. The construction QC record storage facility does not meet general industry fire protection guidelines as promulgated by ANSI Standard N45.2.9, " Requirements for Collection, Storage and Maintenance of Quality Assurance for Nuclear Power Plants". It appears that the licensee is not committed to this criteria. Therefore, as directed by memorandum from G. W. Reinmuth, IE:RCI to C. E. Murphy RCES Branch, RII dated October 5,1979 this item is not an enforcement item and is closed.

h.

(Closed) Inspector Followup Item (416/79-10-08): Inspection, test and maintenance of fire protection systems.

Temporary procedures have been issued for inspection and test of the fire protection systems until the permanent plant procedures are issued.

The procedures include the following:

06-S-01-P64-Temp 8, Fire Pumps (weekly);

06-S-01-P64-Temp ill, Fire Protection System Valve Lineup Verification (wwekly); and, 06-5-01-P64-Temp 12, Fire Protection Deluge and Sprinkler Systems (weekly / monthly). This item is closed.

(Closed) Inspector Followup Item (416/79-10-10 and 417/79-10-06):

Combustible scaffolding.

The licensee advised that the use of combustible scaffolding materials in the plant is less than 10% of the total scaffolding in use. This is in conformance with the insurance carrier's requirements.

During the plant tour the inspector noted that the use of combustible scaffolding materials were very low. The licensee advised on November 10, 1980 that the Bechtel Procedure on housekeeping for Nuclear Power Plants, had been revised to incorporate restrictions on the use of combustible scaffolding materials.

Therefore, this item is closed.

Permanent Plant Fire Protection System Installation a.

Plant Tour A tour was made of the plant by the inspector for an inspection of the following fire protection features:

(1) Fire Pumps An inspection was made of the fire pump installation.

The electric driven fire pump is supplied power from a motor control center located in the water treatment building.

The circuit breaker in the power supply circuit to the electric fire pump is rated at 600 amps with the main circuit breaker for the switchgear unit rated at 1600 amps. The swtichgear unit has a total of 6 circuits which are rated at 600 amps each. Table 9A-1, Item E.2.c of the FSAR states that the fire pump installa-tion will comply with the provisions of National Fire Protection Association (hTPA) Standard 20, Centrifugal Fire Pumps. Section 6-3.4 of hTPA-20 requires the power supply protective devices (circuit breaker or fuse) installed in the fire pump feeder circuit to be rated for the locked rotor current of the pump motor. Also, the power protective devices installed in the power supply circuit at the load distribution center ahead of the fire pump feeder circuit is required to be rated at least at the sum of the locked rotor current of the pump motor and the maximum load currents of the other circuits protected by this device.

The electric fire pump is driven by a 150 HP 480 VAC motor which has a locked rotor current rating of approximately 1050 amps.

The failure of the fire pump installation to meet the criteria of NFPA20 as committed in the FSAR is identified as Deviation Item (416, 417/80-25-02), power supply for electric fire pump motor is not designed for locked rotor current.

(2) Fire Protection Yard Piping System The underground piping system is pratically complete. A review was made of piping inspection and hydrostatic test documents.

This review is discui: sed in paragraphs 5.b.

(3) Carbon Dioxide Extinguishing Systems The installation of the carbon dioxide systems is in progress. A review was made af the piping inspection and pressure test documents. This is discussed in paragraph 5.b.

A detail inspec-tion of these systems will be made during a subsequent NRC inspec-tica after the f.nstallations are complete.

(4) Automatic Sprinkler Systems

-5-Refer to paragraph 3.c for details on installation discrepancies for the sprinkler systems being installed in the diesel generator building. The licensee advised that the sprinkler systems are to be inspected by a field engineer for conformance to the construction drawings and specifications and are to be inspected by the plant insurance carrier for conformance to the NFPA and insurance criteria. These systems will also be reviewed during a subsequent NRC inspection.

(5) Fire Detection Systems A field engineering group is inspecting the installation of the fire detection and alarm systems to assure conformance to the construction drawings and specification. The installation of the systems for Unit 1 are scheduled for completion by the spring of 1981. These systems will be inspected during a subsequent NRC inspection.

(6) Fire Dampers The fire dampers for the safety related ventilation systems, such as the control room HVAC system, do not appear to be properly installed. Some of the horizontal dampers are not attached on all four side of the damper to the duct system and most of the ducts are not attached to the building walls. Also, some of the vertical fire dampers do not appear to be attached to the floor assembly. The inspector reviewed the construction documents and noted that the design data did not indicate specifically how the fire dampers _were to be installed. In addition, neither vendor drawing nor construction installation drawing or procedures were available to indicate how the dampers were to be installed. The subcontractor installing the dampers is apparently trying to accomplish a satisfactory installation.

However, without approved fire damper installation drawings there is no assurance that the dampers will be properly installed. The licensee has issued a "stop work" for installation of any futuer dampers until adequate design and/or installation drawings are provided.

Criterion V of Appendix B to 10 CFR 50 states in part that

" Activities affecting quality shall be prescribed by documented instructions, procedures or drawings of a type appropriate to the circumstances...." The failure to provide adequate installation criteria for the fire dampers is identified as Noncompliance (Infraction) Item (416/80-25-01 and 417/80-15-01), substandard fire damper installations.

-6-around each pump. The licensee is reviewing this discrepancy to determine the requirements for these tanks.

This item is identified as Unresolved Item (416/80-25-03 and 417/80-15-03),
