The licensee responded to the Notice of Violation (Notice) on May 16, 1988. Discussions of each item identified in the Notice as well as other open items follow, (Closed) VIO 416/87-32-01, Commercial Grade Parts Installed in EQ Equipmen .

A previous inspection identified commercial grade parts had .been installed on equipment requiring environmental qualification without an adequate evaluation of their suitability for use in safety-related applications. The reasons for the violation were inadequate training for Materials Technical personnel, and inadequate plant procedure Procedure 12-S-01-15, Procurement Evaluation for Environmentally Qualified . Equipment, was issued February 27, 1988, and revised on June 28, 1988. When this procedure was issued, the licensee began an evaluation for commercial grade components installed in EQ equipmen Parts identified to date have included indicating lights, pressure switches, and relays in the Hydrogen Analyzers; Marathon 300 terminal blocks in Limitorque operators; and breaker components in switchgear dnd load control centers. The licensee's approach to this evaluation has been conservative and thorough. The licensee is expanding its evaluation to include all safety-related equipmen Evaluations that were completed prior to the inspection showed that the terminal blocks were qualified but the paperwork necessary to show qualification was not complete. The licensee prepared a Material Nonconformance Report (MNCR) for the terminal blocks which indicated tha'c the terminal blocks were qualifiable, The licensee then elected to replace the installed terminal blocks with fully qualified terminal block The components associated with the Hydrogen Analyzer were qualified during initial testing; however, the replacement parts were procured commercial grade. The licensee has sent samples to the vendor to determine if the spares they have can be qualified, and has also ordered qualified spares from the vendor. The commercial grade components are the same as those purchased by the vendor, however, the vendor performs inspections upon receipt from Westinghouse and ,

dedicates them for use as qualified component )

In all cases identified to date, qualifiability has been shown and justification for continued operation has been documented, or the part replaced and an evaluation performed to show qualifiability of the installed componen The licensee conducted training on the requirements of the EQ Program May 5 and 6,1988, for the members of the Material Technical Group who perform quality level procurement. This training has aided in the identification of potentially unqualified equipment during the review of commercial grade component Procedure 12-S-01-15 did have a statement that could have caused problems in the future. The procedure contained a caution statement ;

A previous inspection identified that the Limitorque' qualification file did not adequately address some actuators installed in the plant in that tne installed configuration was not the same as that teste Specifically, actuators inside the drywell did not have grease reliefs installed, and T-drains on Valve No.1EE30F002B acted as a l

high point vent rather than a drai The licensee issued Material Nonconformance Report (MNCR) 411-87 to evaluate other Limitorques that were not installed in a tested t configuratio Seven additional limitorques were identified as requiring installation of T-drains on the limit switch compartment for low point drains. The T-drains were installed prior to the end of the second refueling outage. Additionally, 13 Limitorque valve within the drywell were identified ar requiring grease reliefs. The grease reliefs were installed prior to the end- of the second refueling outage als The inspector reviewed the MNCR and documentation for inspecting and ,

repairing the valves as necessary. Additionally, the inspector

Limitorque valves had been modified to include T-drains as low point l drains and also ensure grease reliefs for valves inside the drywel Based on the inspection and repair of the identified valve and the !

modification of the EQDP, this item is considered close l (Closed) VIO 416/87-32-03, Raychem Heat Shrink Tubing in Unqualified Configuratio l A previous inspection identified that the Raychem tubing installed on the Hydrogen Recombiners was not in accordance with a tested conf!guration. The unqualified splice was replace shortly after the inspection with a fully qualified four-to-one Raychem ki This was the only instance of a problem with splices identified during the EQ inspection. There was another item from a previous inspection that was added as an example to this violation. That example was non-standard Raychem heat shrink tubing installations identified during the licensee's response to IEN 86-53. A review of I

the licensee's response to IEN 86-53 indicated that all questionable splices had been replaced with fully qualified splice As a result of the additional finding during the EQ inspection, the licensee reviewed and rev' sed several procedures. The inspector reviewed the following procedures and found them to be acceptable:

07-S-12-109 Environmental Sealing of Class 1E Equipment, Rev. 2, 4/27/87 Installation Standard ES-10 for Class IE Equipment Environmental Sealing, Rev. 5, 5/11/88 Installation Standard ES-04 Electrical Terminations and Splices, Rev. 3, 4/23/87, with SCN 88/001, 7/14/88 Additionally, the licensee provided training to all I&C Technicians on the proper installation of Raychem. These technicians had not previously had training on Raychem applicatio Based on the results of the response to IEN 86-53, the procedure revision, the training, and the replacement of the Hydrogen Recombiner splices, this item is considered close (Closed) VIO 416/87-32-04, Qualification Package for Lubricants A previous inspection identified that the licensee did not have the documentation necessary to show qualification for Mobilux EP-2 grease. As result of this findings, the licensee issued EQDP 35.1, General Purpose Lubrication, which was reviewed by the inspector and shows qualification in accordance with 10 CFR 50.4 The licensee also revised Engineering Standard ES-19 for Environmental Equipment Qualification Maintenance on June 30, 198 l This revision specifies the type of Lubricant to be use '

Based on the issuance of the EQDP and the revision to ES-19, this I item is considered close ' (Closed) URI 416/87-32-06, Accuracy Calculations for PAM Instrumentation for Postulated Harsh Environment A previous inspection identified that instrument accuracy calculations had not been performed for Post-Accident Monitoring (PAM) instrumentation. This was due to the licensee's position that those calculations were not required. The licensee subsequently committed to performing conservative calculations on typical PAM Loops to confirm expected loop accuracies for operator response.

The licensee has been performing calculations manually and has pursued the developement of a computerized system concurrently. The inspector reviewed the following calculations:

The calculations were found to be acceptabl The inspector observed a demonstration of the computerized system for performing the accuracy calculations. The system should provide the required calculations in an acceptable format. The licensee is tracking the completion of the accuracy calculation un its Licensing Commitments Tracking System (LCTS) and has assigned LCTS ID NO. 15343 a completion date of July 1,1989.

6 (Closed) URI 416/87-32-08, Seal Assembly for Victoreen Radiation Monito A previous inspection . identified that the cover cap on the junction box was on hand tight without a gasket, 0-ring, or other apparent sealing devic The licensee measured the cap and verified that the threads were tapere Calculations were also performed to demonstrate the structural integrity of the enclosure under accident condition A Maintenance Work Order was performed by the licensee to verify the taper and thread engagemen The work order called for thread sealant to be applied when re-installing the cap. The "as found" condition stated that there was greater than three thread engagement and sealant. The Work order required a minimum of three thread engagement with the thread sealant applied. The "as-left" condition had greater than seven thread engagement with the thread sealan Based on the facts that the threads were tapered and had more than three thread engagement and that the licensee added thread sealant as an extra measure, this item is considered close .
