Gentlemen:

! This refers to the inspection conducted by Messrs. D. R. Carpenter,

H. F. Bundy, T. Reis, and other inspectors of this office in conjunction with

4 NRC consultants during the period March 9 through April 10, 1987, of activities

authorized by NRC Construction Permit CPPR-128 and 129 for the South Texas

! Project (STP), Units 1 and 2, and to the discussion of our findings with

. Mr. W. H. Kinsey and other members of your staff at the conclusion of the

i inspection.

Areas examined during the inspection included Technical Specifications (TS)

review, review of the Integrated Leak Rate Test (ILRT) and the Structural

Integrity Test (SIT), preoperational test review, preoperational test results

review, startup testing program review, as-built plant to documentation

,

reconciliation, the residual heat removal / component cooling water (RHR/CCW)

} incident review, operational staffing review, prelicensing review of training

i

and qualification programs, reactor coolant system loss of cleanliness recovery

program, Three Mile Island (TMI) and generic letter (GL) 83-28 action items,

licensee action on previous inspection findings, site tours, review of the

manual trip circuit, and procedure review.

. Within these areas, the inspection consisted of selective examination of

I procedures and representative records, interviews with personnel, and

,

observations by the NRC inspectors. The inspection findings are documented in

the enclosed inspection report,

i During this inspection, it was found that certain of your activities were in

4 violation of NRC requirements. Consequently, you are required to respond to

j these viciations, in writing, in accordance with the provisions of

i Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10 Code of

] Federal Regulations. Your response should be based on the specifics contained

<

in the Notice of Violation enclosed with this letter.

You are requested to respond, in writing, to Open Items 498/8708-34,

498/8708-61, and 498/8708-62 in Section 16 of Appendix B. These open items

concern Licensee Compliance Reviews, documentation of deviations from the

Westinghouse Owners Group Emergency Response Guidelines, and exceptions to the

emergency procedures writers guide. These and the-other open items need to be

dispositioned prior to fuel load.

We have also examined actions you have taken with regard to previously

identified inspection findings. The status of these items is identified in

paragraph 13 of the enclosed report.

The response directed by this letter and the accompanying Notice is not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.

Should you have any questions concerning this inspection, we will be pleased to

discuss them with you.
