Allegation (Closed) Allegation (RIII-86-A-0003): On January 6, 1986, the Region III Enforcement and Investigation Coordination staff received information from the Office of Investigation (OI) pertaining to Magnetic Particle Examina-tions (MT) that were performed at the Callaway Plant. The alleger stated that the Daniels International Technical Service night shift superintendent (known only as Bob) would just throw the magnetic particle powder around the welds and say, "That it was Good Enough." According to the alleger, the individual would at times skip the weld altogethe The alleger thought that the individual may have falsified those MT reports. The allegers direct knowledge of the individuals activities was limited to the last week of May 1984, when the alleger assisted the individual in conducting 40-50 MT's on supports in the auxiliary building. The alleger also stated that he thought that the former dayshift NDE supervisor who now resides in Lake of the Ozarks, Missouri could fully identify Bo ,,

NRC Review The NRC inspector interviewed three UE individuals that were supervisors in QA, an individual that was an engineer for Daniels, and the Authorized .

Nuclear Inspector that was onsite in May 1984. During these interviews i the NRC inspector was informed of the name of the town where the former l Daniels dayshift NDE supervisor lived. The NRC inspector telephoned the individual and he stated that while he was working at the Callaway Site he knew two Bob's and that they were both excellent inspectors. He did not know of a superintendent named Bob. He stated that to the best of his knowledge everyone followed procedures and that all the NDE was performed properly. He also stated that the MT in the auxiliary building was completed prior to May 1984. He indicated that his home is near the Callaway Plant and that he didn't mind living there because Callaway is a safe plan I In reviewing Daniels QC and construction organizational charts and training records it was determined that there were two superintendents working for Daniels construction in May 1984 that had the first name of Bob. One individual was an insulation superintendent and the other individual was an iron worker superintendent. The superintendents were not qualified in nondestructive examinations (NDE). The Daniel Quality Organization, which included the Technical Services Lab (TSL), did not have any superintendent There were approximately 7,500 Q (safety-related) and non Q (non safety-related) supports in the auxiliary building. All the supports in the auxiliary building are ASME Class 2 or Class 3 and only required a Visual Examination (VT). The VT's were performed in accordance with ASME Section III, 1974 edition 1975 addenda. All welds attaching supports, lugs, brackets, stiffeners and other nonpressure attachments to pressure parts on all exposed surfaces were Magnetic Particle (MT) or Liquid Pene-trant Examined (PT). These tests were properly performed and were performed by qualified personnel. The site staff performed a record search back to November 14, 1983, for the NRC inspector, and found that there was no MT performed on support attachments in the auxiliary building from November 14, 1983, to the present. In reviewing the ASME N5 data reports, it was revealed that the last three systems in the auxiliary building were code stamped the first part of May 198 The NRC inspector reviewed Daniels MT and weld repair procedures, all the Daniels Technical Services Lab (TSL) NDE personnel certifications that had the first name of Bob or Robert, that MT during certification May 1984, and approximately one third of all other NDE personnel that were onsite from 1977 through 1984, and found them all to be properly qualified and certified. Additionally, the NRC inspector reviewed the certifications of the QC inspectors that had the first name and surname of Bob, Robert, Rob, Ron or "R" and that was onsite in May 1984. Based on the foregoing, the NRC inspector did not substantiate th6 allegation. The Daniel superintendents were in construction and were not qualified in NDE. Also, there were no superintendents in Daniels Technical Services Lab. All NDE personnel were properly qualified and certifie All supports in the auxiliary building were Clae 2 or 3, and only required a visual examination. There wasn't a MT performed on a support attachment in 198 . Exit Interview The inspector met with licensee representatives (denoted under Persons Contacted) at intervals during the inspection period. The inspector suninarized the scope and findings of the inspection. The licensee representatives acknowledged the findings as reported herein. The inspector also discussed the likely informational content of the inspection report with regard to documents or processes reviewed by the inspector during the inspection.
