Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/88-19 AND 50-328/88-19)

This refers to~ Nuclear Regulatory Commission (NRC) inspection conducted by

Kenneth Jenison on March 6,1988 thru 3pril 4,1988 and April 15, 1988. The

inspection included a review of the activities authorized for ynur Sequoyah

facilitiy. At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the resort. These areas

, include operational safety verification and review of open items, violations

and licensee event reports.

The inspection findings indicate that certain activitie, appeared to vinlate

NRC requirements. Violation A involves a failure to install the shield

building penetration seals in accordance with the drawings. Violation B

involves a failure to have ar, adequute surveillance instruction to ins)ect fire

barriers. Accordingly, the violations, references to pertinent requ rements

and elements to be included in your response are described in the enclosed

Notice of Violation.

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a co)y of this letter, its enclosures,

and any reply will be laced in the NRC's Pualic Document Room.

The responses directed by this letter and its enclosures are not subjected to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
