( 1. '0BJECTIVE 1.1 To provide a procedure outlining the duties and responsibilities of the Duty Officer in the event of an actuation of the Reactor Protection System or the Engineered Safety Features System as required by:

2. PREREQUISITES

2.1 An unplanned actuation of the Reactor Protection System or the Engineered Safety Features System has occurred.

3. INITIAL CONDITIONS ,

4. PRECAUTIONS Performance of this review shall not distract the Control Room staff from maintaining the plant in a safe condition. ,
5. PROCEDURE 5.1 The Shift Supervisor shall start this procedure as soon as possible after stabilizing the plant after a trip or ESF L actuation.
5. 2 The Duty Officer shall verify that the plant is being

maintained in a safe condition. -

I 5.3 The Duty Officer shall verify that any notifications required

[ by EPIP 4701 have been made.

5.4- In the event of a reactor trip, review the following to

. determine the cause and to verify that all safety systems functioned as required. Complete Ops Form 3263-1 and 3263-3.

/ 5.5 In the event of an ESF Actuation, complete the appropriate ESF

Actuation Checklist, Ops Forms 3263-1 through 5. Use the SP-3J10 sequence of events Trip Buffer printout to verify all safety system components functioned as required.

5.6 Conduct with the Shift Supervisor, a complete debriefing of

-involved personnel before they leave the site. Have them complete OPS Form 3263-2.

5.7 Complete an initial Plant Incident Report to include: (use OPS Form 3263-2) a) A chronological description of the pertinent events leading up to the trip (or actuation) and after the trip (or actuation) until stable plant condition is reached.

b) A Comparison of plant response after a trip to the ranges for those parameters given in E0P E-0/E-1/ES 1.1. Comment .

on any deviations.

c) the cause of the event d) a determination as whether all safety equipment functioned

( properly e) corrective action, if any f) action taken to prevent recurrence, if any.

5.8 Attach all completed Ops Forms to the PIR. Include the completed Safety Function Status Check Forms.

5.9 Attach a copy of the analog prepost trip review, sequence of events printouts and copies of any pertinent strip charts.

5.10 Forward the PIR to the OPS Supervisor / Unit Superintendent.

5.11 If the cause of the event has been determined and all safety equipment functioned properly, then obtain permission to start up or continue operation in accordance with the Operations Supervisor or Unit Superintendent instructions.

5.12 If the cause of the trip is not known, the Duty Officer will convene a meeting of the Plant Operations Review Committee (PORC) prior to restart to review the trip and transient data.

The PORC review will consider the following:

5.12.1 The actual or most probable cause of the trip.

5.12.3 Additional monitoring or. trending required during ,

5.13 If any safety related equipment failed to function properly, the puty Officer will verify safety related equipment is operable per Technical Specification Requirements for appropriate p40de changes.
