This special report is submitted in accordance with LaSalle Unit 1 Operating Ucense NPF-11, Appendix A, and Technical Specifications 4.8.1.1.3 and 6.6.C regarding Unit 1B Diesel Generator (E22,1E22S001), manufactured by Electro-Motive Division of General Motors Corporation (Model EMD 20654E4).

On June 7,1995, with LaSalle Unit 1 at 100% power, the Unit 1B Diesel Generator (DG) was started in accordance with LaSalle Operating Surveillance LOS-DG-M3, "1B(28) DG Operability Run", for a monthly -

operability run. Approximately 10 seconds later, the 1B DG tripped and the IB DG Overspeed Trip Annunciator illuminated. Further investigation by System Engir'eering and Operating personnel identified an actual overspeed i trip. This failure was not a valid failure in accordance with Regulatorv Guide i 1.108 and Technical Specification 4.8.1.1.3. j I

The cause of the 1B DG trip was operator error. An Equipment Operator-in-Training was operating the fuel racks for the first time during the initial engine warm up period. Instead of pulling back on the fuel racks to slow the engine l down to allow for a slow warm up of the engine, he inadvertently pushed in on the fuel racks thus overspeeding the engine. After the trainee was counseled on the event, the surveillance was subsequently performed satisfactorily. Upon rese; ting the overspeed trip lever and the lockout relay, the 1B DG was successfully operating in accordance with LOS-DG-M3 and retumed to service.
