On November 16,1998, at 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br />, it was discovered that a contract individual had been granted unescorted access to the protected area (PA) without a negative drug test. The individual was placed on the Denied Access list on November 9, 1998, for a presumptive positive Fitness For Duty test at the Braidwood Station. The individual was granted unescorted access on November 9, 1998, at LaSalle County Station.

The unauthorized individual was escorted out of the PA and his PA access was cancelled. Corrective actions included changing the badge control process, providing lessons learned to the other Comed nuclear stations, and conducting a review of all individuals placed on the Denied Access list for the previous thirty (30) days. The safety significance of this event was minimal. After review it was determined that the individual did not perform any work on safety related equipment. There have been two previous occurrences within the past two years. This event was reportable per 10 CFR 73.71(b), Appendix G as an actual entry of an unauthorized person into a protected area.

3. DESCRIPTION OF EVENT I J

On November 16, 1998, at 0845 hours0.00978 days <br />0.235 hours <br />0.0014 weeks <br />3.215225e-4 months <br /> the LaSalle Station Security Administrator  !

was notified by telephone by Nuclear Generation Group (NGG) Fitness For Duty coordinator that unescorted access for a contract individual on the Personnel Denied Access (PDA) list had been granted at LaSalle County Station. Access was granted on November 9, 1998, after the individual was screened and had met all of the Access Authorization Requirements.

On November 3, 1998, the contract individual was randomly tested under Fitness For Duty at the Braidwood Station. The individual's sample was sent to the lab for further testing and was determined to be presumptive positive on November 6, 1998. Notification was made to the Medical Review Officer (MRO) on the morning of November 9, 1998. Upon notification the MRO attempted to contact the individual at the Braidwood Station and was informed that the individual had ,

already been laid off. The MRO office then placed the individual on the PDA and sent the individual a registered letter.

At 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br /> on November 9, 1998, the individual reported for work at LaSalle Station. Screening had been completed and showed that the individual was current at the Braidwood Station and all training needs had been met so the individual was processed for badging.

At 1136 hours0.0131 days <br />0.316 hours <br />0.00188 weeks <br />4.32248e-4 months <br /> on November 9, 1998, NGG Security faxed a list of individuals.that were being added and/or removed from the PDA to the Badge Fabrication Supervisor.

Upon receipt of the fax the supervisor reviewed the additions against the active badge file and did not find any of the additions in the active database. The Badge Fabrication then updated the computerized PDA List and proceeded to screen i in-coming personnel. Since the involved individual had already been screened his record was not re-checked against the additional list.

The. individual entered the PA each day between November 9 and November 14, then again on November 16, 1998.

-On November 14, 1998, the individual reported to the Post Office and picked up I his registered letter from the MRO.

Upon reporting to work on November 16 the individual attempted to contact the MRO l as instructed in the letter. The MRO was unavailable and the individual talked  !

to'the NGG Fitness For Duty Administrator and informed her that he was currently working at the LaSalle Station. j i

This was determined to be a security reportable event in accordance with' )

10 CFR 73.71(b); Part 73, App. G, paragraph I (b). The Shift Manager was notified '

of the event. An ENS notification was made to the NRC on November 16, 1998, at 0941 hours0.0109 days <br />0.261 hours <br />0.00156 weeks <br />3.580505e-4 months <br />.

C. CAUSE OF EVENT The root cause of the event was a Program to Program Interface Deficiency in that the prescreening process was completed on Friday, and not reverified on Monday. ]

This was not in the procedure at the time, and caused this event to occur.

Contributing factors include: (1) different actions requested by two or more l programs for given situation as the PDA is received via fax and the badge fabrication personnel are required to check active database then transfer the list to a site database, (2) inadequate program design in that the PDA is loaded into a computerized database that does not prevent the loading of a badge for an individual on the PDA; and (3) a program-work-planning process in that the PDA j was not re-verified immediately prior to loading a badge into the security j computer.

D. SAFETY ANALYSIS l The' safety significance of this event was minimal. The individual was a pipe insulator and performed no safety-related work, although he did work in a vital area of the plant. Following his removal from the site, his management performed an evaluation of his work, including a walk down of the work area to identify any i abnormalities. No problems were found.

F. PREVIOUS OCCURRENCES

Performance Error There have been two similar events in the past two years. The first event  ;

occurred on April 10, 1998, and involved a contracting company's failure to notify the licensee of the termination of one of their employees who subsequently i entered the PA on three separate occasions. The second event occurred on June 29, 1998, and involved an individual performance failure when a contracting company notified the licensee of the termination of an employee and the badge fabrication supervisor failed to take appropriate actions upon notification. The corrective actions for these events would not have precluded the current event, because the previous two events were Human Performance Errors and this event was a programmatic failure.
