I Subjtet: EARTHQUAKE Th2 licensee notified the Headquarters Duty Officer via the Emergency Notification System at 12:05 p.m. (CST) January 31, 1986, that an earthquake had been felt at the Perry plant. !
Tha United States Geological Survey said the quake registered 5.0 on the Richter Scale, with the epicenter about 30 miles northeast of Cleveland. 3 Rigion III activated its Incident Response Center at 12:15 p.m. (CST), and conferred with
, Headquarters personnel, the Perry resident inspectors, and the licensee.
t Th2 licensee reported no significant damage because of the quake. Some site personnel were svacuated. The licensee also declared a Site Area Emergency, although this was not n:cessary since the facility has not received an operating license; therefore, its emergency l
plan is not yet in effect. Fresh fuel is stored on site, but has not yet been loaded into the reactor. There is no indication of any structural damage to areas where the fuel is i stored.
Licensees at other plants in Ohio and Michigan notified the NRC of the earthquake. Detroit i Edisen said that passive seismic monitors at Fermi did register the quake, but the active i ;
monitors did not. There was no damage. The Davis-Besse, D. C. Cook, and Palisades plants had na " registered" ground movement, and no damage. However, personnel at Davis-Besse and D. C. Cook reported " feeling" ground mover.1ent.
i Regicn III also contacted material licensees in the affected area who handle large quantities of radioactive material. None reported damage.

moderate earthquake was felt at the Perry Nuclear Power Plant.
Shortly after noon a Site Area Emergencyfwas declared by Cleveland Preliminary information Electric Illuminating Company officials.
indicated that the earthquake measured approximately 5.0 on the Richter Scale and the epicenter was located near Chardon, Ohio about 15 miles southwest of the Perry Nuclear Power Plant.
The event was downgraded to an alert at 1:00 p.m. and the event was terminated at 2:25 p.m. There is no damage to major plant systems and no injuries. Nuclear fuel is located in containment in the upper pools and in the fuel handling building, but no fuel has been loaded into the reactor.
An , Incident Response Team has been dispatched by the NRC and will be at the plant tomorrow. February 1.

i As a result of the recent seismic event, it has become necessary to place controla in effect to identify, monitor, document, and rework any damage which might have been caused. All abnormal or unusual observations shall be documented by a Work Request, and EQL be reworked, repaired, adjusted, or otherwise disturbed prior to Engineering review and disposition. All Work Requests will be screened by designated personnel in the PWC for relationship to the seismic event.
The following guidelines shall apply:
When first arriving at the jobsite, survey the area for any visible damage. This includes sticking or binding doors, covers, or other movable items around or near the l device to be worked on.
, During the disassembly or removal of covers, etc. on the i device worked on, be especially aware of any abnormal phenomenon, which may not have been experienced prior to the event. This includes, but la not limited to excesalve friction, loose or misplaced parts or wires, and wetness ,
l not expected to be evident under normal conditions. '
) Additionally, as-found data is now even more important to aid in the evaluation of potential seismic damage. This data can be used to determine the presence of calibration / trip point shift ;
l due to the event, above and beyond the normally expected instru-ment drift. In this light, the following guidelines shall be ob-1 served during the taking of as-found date.
l During the taking of as-found date, it la required that a j second person observe the measurement of the data. I&C l Engineers will be placed on-shift to seeist in this j effort, however they are only required to be present for SVI activities. In other cases, a second technician should be present to observe the data taking. The purpose

**g ' General Supervisors PHONE S740 mooM SB306 Project Organization s ue.J act Work Control As a result of the recent seismic event, it has become necessary to place controls in effect to identify, monitor, document and rework any damage which might have been caused. All abnormal or unusual observations shall be documented by a Work Request (WR) and NOT be reworked, repaired, adjusted or otherwise disturbed prior to review and disposition. All WR's will be screened by designated personnel in the Project Work Center (PWC) for relationship to the seismic event.
The following rules shall apply during the performance of work activities whether controlled by surveillance, maintenance instructions or other documents.
- 1. All Incoming WR's:
The PWC shall assemble a team of structural and system engineers to assess all WR's.
- a. If potentially or actually earthquake-caused --
these will be coded, analyzed and disposition negotiated with the NRC.
- b. If not earthquake-caused -- process and work via the Plan of the Day (POD) .
- 2. Directions to Fields
- a. No work other than POD shall be allowed unless approved by project management (MRE, AX, MDL, FRS, RAS) .
- b. Any work being perfonned requires an observation of the equipment and area as described in #5 below. Any abnormal
' conditions shall be documented via WR's.
- c. In the course of performing work, if something abnormal is found then STOP, document via WR and await further direction.
- 3. Surveillances (SVI) :
- a. ALL SVI's shall be performed only if they are on the POD.
- b. Any SVI activity shall be preceded by an observation of the area as described in #5 below and any abnormalities identified with a WR.
- 4. Preventative Maintenance:
Repetitive Task Cards shall control preventative maintenance activities as before, however, the actions identified in 2b and 2c must be taken.

- 5. a. When first arriving at the job site, survey the area for
! any visible damage. This includes sticking or binding doors, covers or other movable items around or near the device to
. be worked on. ;
- b. During the disassembly or removal of covers, etc. on the device tested, be especially aware of any abnormal phenomenon which may not have been experienced prior to tns event. This includes, but is not limited to, excessive friction, loose or misplaced parts or wires, and wetness not expected to be evident under normal conditions.
- c. Additionally, as-found data is now even more important to aid in the evaluation of potential seismic damage.
Any deviation from the above criteria shall be cause for severe disciplinary action, including dismissal.
