, On. January 9,1993, the unit was shut down to identify and replace a leaMng -

fuel bundle and t,o perform other maintenance activities.

All fuel bundles will be tested for leakage, and any leaking bundles will be replaci. The outage is expected to last about 40 days.

Evidence of one or more leaking fuel bundles was detected in elevated offgas measurements in mid-1992. The offgas measurerents have been gradually increasing, but are still well within HRC release limits. The licensee has also experienced higher airborne radioactivity levels than normal and i,hort-lived' contamination in various parts of the ply L

  • The licensee's fuel testing and maintenance activities will be monitored by the resident inspectors and inspection specialists from the Region III (Chicago) 6 ',

office. One of the major activities being monitored will be local leak rate testing (LLRT) of the main steam isolation valves (MSIVs). The LLRT on the HSIVs is being performed as a result of previous problems in this area andV'a mM coramitment with the NRC to perform the testing.

The State of Ohio will be notified. The information in this preliminary notification has been reviewed with licensee management.
