-SPECIAL REPORT NUMBER 89-07 Fire Barrier Breached More Than 7 Days Date of Occurrence March 4,.-1989 Plant Conditions at Time of Occurrence On_ January 31, 1989, Control Room Operators conducted a mandatory shutdown of the plant as required by Technical Specifications due to an inoperable.

Auxiliary Feedwater System. The plant was placed in cold shutdown on February 1. Associated investigation and repairs resulted in a plant outage until restart on March 13, 1989.

Identification of Occurrence Fire barrier door AU 133 was not restored to operable status within the 7 days required by the Rancho Seco Technical Specifications.  !

Description of Occurrence On February 25, 1989, security personnel discovered that fire barrier door  !

AU 133 was out of alignment. Work Request 149850 was initiated to repair the j fire barrier door in accordance with Maintenance Procedure H.100 " Fire Barrier  !

Door Maintenance. - Repair of the door was the responsibility of the  !

Facilities organization. The failure to meet the 7 day requirement was due to '

the commitment of qualified welders in the Maintenance Department to outage activities, and delays due to commitment of Planning personnel on outage activities and a revision of procedure H.100.

H Corrective Action Taken An hourly firewatch was previcusly posted in the area and satisfied the Technical Specification 3.14.6 requirement for the deficiency. The revision to Procedure M.100 was completed prior to initiation of door repair. The fire l

barrier door was restored to operable status on March 7, 1989. As a result of this occurrence the Facilities organization has qualified two of their personnel as welders.
