Engine Servicee hereby notifies the Commiceion of a potential defect in a component of a DSB Standby Diesel Generator.
There could exist a problem with the accessory drive assembly.
Cooper Industries has supplied this component with the potential problem to the following nuclear sitec:

i At Gulf States Utilities, River Bend Station, the two (2)
I bushings in the accessory drive assembly were found to be seized t3 the shaft and rotating in the housing.
This condition was obeerved by Cooper Industries personnel in the course of performing outage maintenance on the standby diesel generators.
The root cause of failure has been determined to be a combination of insufficient lubrication to the bushinge with a stackup of j
machined tolerances all toward one end on the assembly l
componente.
Lubrication of the bushingc is via a 1/4 inch tube from the oil distribution manifold in the gearcace.
Eight (8) other lubricating tubes also originate in thic manifold including one for the gear mesh in thic assembly.
Lubrication for the gear mech is through a 1/32 inch orifice spray jet; an oversize orifice here may cause a reduction of oil flow to the buchings.
The combination of tight tolerances and reduced oil flow could cause overheating of the bushinge and ultimately bushing failure.

The accessory drive assembly is designed and manufactured b/
Cooper Industries, Enterprise Engine Services, located in Oakland, California.
Affected assembly part numbers include 1A-3541 and 1A-6877.
Cooper Industries recommends the following cource of action at the next scheduled engine maintenance.
i 1) a visual inspection (including barring the engine over) l to detect spinning bushings, j

2) an inspection of the orifice spray jet to be sure it is l
not oversized, and J
3) continued surveillance according to the DR/QR suggeeted l
maintenance schedule.
Since action is required by others, we cannot estimate when corrective action will be complete.
A copy of this letter will be forwarded to all of the affected sites refelenced in paragraph 2 of this letter as indicated by the carbon copy list.
Our evaluation of this matter was concluded on June 13, 1989.
