SPECIAL REPORT NUMBER (4 16 Post Accident Sampling System Inoperable More Than 72 Hours Date of Occurrence September 18, 1988 Plant Conditions at Time of Occurrence The plant was operating at approximately 92% power on September 18, 1988.

l Jdentif tatiga of Occurttatt The Post Accident Sampling System was not returned to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> as required by Rancho Seco Administrative Procedure RSAP-0219.

Descriotion of Occurrence On September 15, 1988, a work clearance was issued to allow removal of Pressure Control Valve PCV-70022 for maintenance.

PCV-70022 provides pressure control for the Post Accident Sampling System dilution / flush water from the Miscellaneous Water Holdup Tank T-993.

To isolate PCV-70022 clearance tags were placed on additional vgives which disabled the flow path for the backup dilution / flush water from Chilled Hater Tank T-496. Without a source of dilution / flush water the system became inoperable.

Corrective Action Taken On September 30. 1988, backup dilution / flush water capability from T-496 was restored to return the system to operable status.

Restoration of the normal dilution / flush water capability from T-993 will be accomplished on receipt of parts for rework of PCV-70022.
