Fire barrier doors NS 155, NS 238 and NS 240 were not restored to operable status within the 7 days specified in the Rancho Seco Technical Specifications. The Sacramento Municipal Utility District hereby submits Special F, rt No. 88-17 in accordance with Rancho Seco Technical Specifica sn 3.14.6.2.

SPECIAL REPORT FUMBER 88-17 Fire Barriers Breached More Than 7 Days Date of Occurrence October 26, 1988 Plant Conditions at Time of Occurrence The plant was at approximately 90% power on Octooer 19, 1988. Control Roou Operators conducted a controlled shutdown in accordance with Technical Specifications on October 24. The plant was in hot shutdown on October 26.

Identification of Occurrence Fire barrier doors HS 155, NS 238 and NS 240 were not restored to operable status within 7 days as required by Technical Specification 3.14.6.2.

Descriotion of Occurrence On October 19, 1988, during the performance of Surveillance Procedure SP.1004, "Refueling Interval Fire Barrier Surveillance Procedure," techricians discovered the indicated discrepancies on the associated fire doors:

  • NS 155 - deficient closer mec'hanism
  • NS 238 - nonoperable door latch
  • NS 240 - nonfunctional door coordinator All three doors are locate'd in the fluclear Services Electrical Building.

Corrective Action Taken An hourly firewatch was previously posted in the area and satisfied the Technical Specification 3.14.6 requirement for the deficiency. The fire barrier doors were restored to operable status on October 31, 1988.
