Dear Mr. Martin:
Fire barrier penetrations X41105, X41105LB, and X44296, were not restored to operable status within the 7 days required by the Rancho Seco Technical Specifications. The Sacramento Municipal Utility District hereby submits Special Report No. 89-04 in accordance with Technical Specification 3.14.6.2.

Date-of Occurrence February 16,'1989 Plant Conditions at Time of Occurrence The plant was at c'ld shutdown.
o Identification'of OccurIRace Fire barrier penetrations X41105, X41105LB, and X44296 were not restored to operable status within.the 7 days required by the Rancho Seco Technical Specifications.
Description of Occurrence The fire barrier penetrations ~were breached'during new construction on the Non-Nucinar Instrumentation (NNI) under Work Request 149681. At the same time, extensive work was being performed associated with the evaluation and repair of the Auxiliary Feedwater (AFH) System following the' January 31 AFH System overpressurization..The decision was made to continue the assignment of resources resolving the overpressurization issue since compensatory action had been taken by posting an hourly firewatch. As soon as the AFH evaluation tasks were completed, the fire barrier penetrations were restored to operable
-status.
_orrective Action Taken C
An hourly.firewatch was previously posted in the area and satisfied the Technical Specification 3.14.6 requirement for the deficiency. All-of the fire barrier penetrations were restored to operable status on February 18, 1989.
